Prepare for Your Visit

Typing the items below on a word doc is preferred to handwritten.

  1. It’s your responsibility to Bring all medical records preferably on a flash drive but if not bring paper records. DO NOT depend on the other doctor offices to send records. Start gathering these records at least 1 month before your appointment and bring them in by hand the day of the appointment. We need all important Radiology reports (MRI, DaTScan, PETScan) and CD’s. You can get the CD’s of the images where you had your scan done.

  2. The week before your visit make a medication list on your computer or use the BPMedTracker App to print a medication list. Make sure you have the Name, dosage, how many pills how many times a day (not just lumping all pills into one daily dose if not taken all at once), time you take each pill. Also bring all medications in case you make a mistake on your list or something is not clear. Not having a medication list or medications during a visit will cause delays increasing other patients wait times. Do this even if you update them in the patient portal. Include the name of your pharmacy.

  3. Type out all of your Past Medical History (what you have been diagnosed with in the past or that has resolved), All surgeries, Allergies, and family medical history of your grandparents, parents and siblings

What to expect at your first visit

  1. Before your visit

    Fill out your patient forms. Bring a list of all your medicines and doses, your glasses and hearing aids, and any past test results, brain scans, or records.

  2. Bring someone who knows you well

    A family member or friend can share what they have noticed and help you remember what we discuss.

  3. A careful evaluation

    We talk about your symptoms and history, do a neurological exam, and do cognitive testing. The first visit takes longer than follow-up visits.

  4. A plan you understand

    We explain what we found, order any tests you need, and build a plan with you and your family. We see you again over time to track changes. Telehealth follow-up visits are available for established patients.
